             I, Ruth Smith (nee Ruth Doe), being first duly sworn on oath depose and say as follows:

1.      That I am the daughter of the decedent, John Doe, who died in Melrose Park, County of Cook, State of Illinois on March 15, 2002. 

2.      The decedent having been married once to Marilyn Doe (nee Brown), she having predeceased decedent, had four (4) children born and none adopted, namely:

a.      Ruth Smith (nee Doe), Affiant

b.      John Doe, son

c.      Tom Doe, son

d.      Henry Doe, son

3.      The approximate value of the decedent’s estate both real and personal is $ 200,000.00.

4.      The decedent died testate.

5.      Therefore, John Doe left surviving him Ruth Smith (nee Doe), Affiant, John Doe, son, Tom Doe, son, Henry Doe, son, as his only heirs. 

FURTHER AFFIANT SAYETH NOT.
